relay.nhs.uk will only accept incoming mail from machines with a DNS
entry. So, for instance all of our mail comes from our relay at
smtp.whalebridge.nhs.uk
Now this was only introduced about 18 months ago and so all machines
that sent mail in the period before the changeover were given a sort of
DNS record to make sure that the service did not fall over. I would
strongly suspect that the machine that does not work did not send mail
before the cut off date.
A reverse DNS lookup on you machines IP address with reveal the truth or
otherwise of this theory (email me the IP addresses if you don't know
how to do that.
