I think paragraph 26 is simply wrong. Rubbish in fact.
Each death results in a packet of notes returning to the Health Authority.
All that is required is a pencil, a piece of paper, and the ability to put a
mark on the sheet against each GP's name.
I suppose decide whether it is better to draw occasional lines indicating the
passage of time periods and start again beyond them or to leave it fully
cumulative is a decision to make, but it hardly qualifies as
