I have already had an instance of a consultant phoning me to say things
that he didn't want to commit to writing in a letter which was being
copied to the patient, but which he nevertheless felt needed to be said.
So then of course I had to keep a record of what was said to me (so that
my partners are aware of it when seeing the patient) so it goes into our
records which the patient may in due course decide to read...
