A common problem where I work is that of failing to prune mailing threads,
and also drift of threads. This occurs where a message is sent to multple
recipients , each of whom has a say - relevant or otherwise, and the thing
snowballs. I've seen some mailings that contain incredible indiscretions
within the threads.
Not an email fault - a user fault as usual

>Many of the flaws relate to human error, and the classic example is someone
who hits a reply to all button, and inadvertently sends a confidential reply
(if such a thing can exist in email) to many recipients. Similarly,
ignorance of bcc has offered us many useful email addresses on occasions.
