I think the problem with labels is that people tend to assume that the label
they have is the label they need. There have been mistakes made using computer
generated information. The classic was patient A blood in tube, patient B
label on tube and patient C labels on form. We have never allowed labels on
cross match bottles and forms and for the next six months are seeing whether
hand writing ALL blood forms and bottles reduces errors. So far it has.

We are not allowed to use the ordinary labels by the lab... something about
not fitting in the centrifuge or not being able to see the blood once spun
down.
So is anyone using labels produced by their computer system... to save on
writing on all those bottles!
