Not a totally uncommon experience, Danny. Such incidents tend to occur in
'fits and starts', in my experience. I've never audited the problem, but
because of its relative infrequency, put it down to someone having a bad
day/week etc.
Probably they are good examples to use in the work of your Emergency
Services Collaborative?
