I nearly understand that, but where is the fourth bracket? And what's with
the the fifteen minute business? And does anyone really know the answer or is
it a bit like getting opinions from counsel, you just keep asking till you get
an opinion you like the sound of? And are there eleven or are there ten
sessions in a week? And what does notional mean in respect of NHDs.

There are (or certainly used to be) BMA recommendations. Basically 5- 7
fixed sessions a week. Evenings considered double time, weekends and nights
triple; on call availability (as opposed to time actually worked- although
particularly if call ins were rare a lower rate might be acceptable in
return for a relaxation of the 15 minute journey time rule so you could be
on call from further away from the hospital or engage in other activities
while on call) working out at 11 NHDs a week divided between however many
were on the rota. No national negotiations on this and unlikely to be so for
some time, but presumably up to the individual to negotiate if any change in
job planning to include fixed unsocial hours session is suggested.
Looking at other specialities, I've come across anaesthetists who do out of
hours sticking strictly to the 3.5 hour NHD, so 2 days of 08:00- 18:30
comprises their 6 fixed sessions for the week.
I agree with Andrew that there is a problem with taking time off in lieu in
that it tends to take you away from meetings (particularly informal meetings
with other specialities and managers dropping in), so you either pull away
from strategic direction (and get seen as being 'never there') or work the
evenings and weekends in addition to office hours.
As Nick says, though, there are other things to negotiate than time off and
money.
