Hypovolaemic shock has important physiological reflex mechanisms and we need to look at the efficacy of administering Prehospital IV fluids, ie blows off clots etc. We need to look carefully at the prehospital interventions in the field. The paper was published in 1996 and was quoted as the "Yellow Cab study". Demetriades's study compared a matched cohort of patients the only criticism was that it was retrospective.
I wouldn't look too seriously at it as if we did similar studies into the medical profession we could probably demonstrate that Doctors and their interventions are more deleterious to health. I remember at the time having a chuckle to myself but didn't worry undully. We have to work together apart from a few prats out there in all professions.
