Frank,
I know who you are and I have see the website. I think it is a very good
idea and very impressive, but that does not take away from the point that
evidence based practice and even guidelines are only that... The
guidelines in australia alll state at the beginning that "these are only
guidelines. Mangement of the individual is the most important." Guidelines
assume homongeneity of the population and if used wrongly, can be
dangerously misleading for the patient. Keep in mind we are dealing with
people who vary greatly! This is what i mean by integrating the literature
with one's clinical expertise and patient care.
Of course I know about the insurance situation etc. However, keep in mind
research in Physiotherapy is still young and there is much to go in terms
of not just providing evidence for efficacy, but also mechanisms by which
Physiotherapy works. Therefore to say anything is conclusive at this stage
is ridiculous.
I respect the fact that everyone on this list are from different
backgrounds and environments and hence have different opinions. As I said,
we don't have to agree, and by god we don't have to always prove others
wrong. But your lack of insight into the evolution of other approaches and
ideas, especially the trend towards neurophysiological explanation in
Manual Therapy and exercise therapy put forth by people like Louis Gifford
and David Butler (and believe me, he has changed a great deal.... try
reading the new book rather than hearing what others say) tell me that you
are still living in the "stone age" using your own words again. McKenzie
Physiotherapy has evolved a lot since the 80's, so do you think other
manual therapy approaches are going to stay static?
I feel that all I am doing is putting forth my views based on my
background and have learnt long ago that there is no right and wrong in
Physiotherapy. However, your strong desire to prove people wrong based on
your collection of literature is unfortunate, especially for someone in
your position. I feel that this debate will only end until you feel that
you have proven me wrong and "wave me off", and that to me is not the
purpose of this forum or why I am here.
Therefore, this is my final post and I am unsubscribing myself out of this
post. Thank you for helping me learn throughout the last 3 years of my
career. Being a new graduate physio when I first subscribed 3 years ago, I
have found it to be a invaluable source of information.
