Staff grades, on the other hand, are tied to a national contract, I believe,
so that their overtime is limited to a maximum of 12 hours, all paid at
standard rate (but please correct me if I'm wrong!).

--> No doubt staff grades who sign a standard "staff grade contract" are
paid as such, but they can nowadays demand to be put on their own
personally-set contract, e.g. as a fellow or as locum at service or whatever
and be paid whatever they are worth to the department.
If there's anyone out there capable of doing a middle grade job, they are
much in demand nowadays...
