I would also keep a copy of the cert in the OH file and send one to HR - the
necessity of this has been proved by managers occasionally claiming that I
had passed someone I hadn't.
We never actually "fail" someone - I believe that's a HR job - instead I
state "fit with restrictions" and detail what modifications (if any) are
needed to support the potential employee in the role (e.g. no manual
handling, no standing, etc) and HR decide if they can accommodate that
"reasonably".
