We quite often advise a phased return but consider that it is for the
employer to decide if they can accommodate it - they nearly always do. While
we may suggest a length of phased return we wouldn't often specify the
precise details of the gradient back to full hours. We leave that as
discussion between employer and employee.
Don't know that there is a hard and fast rule about payment but certainly
anyone in the first six months of absence would be getting full pay for sick
days anyway. After that I suspect most Departments (NI govt) just pay full
rate for days at work and sick rate for those off.

The most important thing about a rehabilitation / therapeutic return is that
it is tailored to help the individual continue their recovery.
Whether HR can implement it is up to them, subject to DDA, H&SAWA, etc!
