I take people attending as consent, although if they have not seen and signed the management referral, I go through that with them and ask them to append their comments and sign the form before I start.
A significant part of the consultation is the individual's report of any attendance or performance issues and the reasons for these - sadly often related to breakdown in communication at work!
We have consent statements on the vaccination record form.
