Cheryl,
You're quite right that midwives should understand the longer term
consequences of childbirth. Unfortunately, a lot of midwives are stuck
on the “hand over at ten days” convention. Extending this to 28 or 42
days would be an improvement, but I found a reluctance to contemplate
the sequelæ of birth when I started looking at postnatal quality of life.
We involved women at 6-8 weeks and 6-8 months after childbirth. Their
experiences should certainly inform midwifery practice, because so much
of it is a consequence of what happened during labour.
