Dave
This is one of the problems with self-registration (see my recent email to
Emma) in that the from doesn't require the person to give job title. This
makes it much harder to authenticate or simply to know who new users in
primary care are.
I haven't yet had an authentication request from Athens (all my
self-registers have obviously used NHS PCs to register), but my approach
would be to email the person and ask. Given the diversity of primary and
social care, she might well be legitimate.
