SUBJECT LINE: Important - For International Disability Service Providers
Dear Listserv Member:
The Association on Higher Education And Disability (AHEAD) in the United
States is conducting an international survey of staff members in
disability services (DS) offices. In the past, these types of surveys
have only included North American DS directors. We are making every
effort to include international colleagues, including those working with
deaf and hard of hearing students. The purpose of this survey is to
collect information about all staff providing services to disabled/deaf
college and university students. This online survey is in English and
should take about 15 minutes to complete. You can access the survey by
clicking on the link below or by using the URL with your web browser
(e.g. Netscape or Internet Explorer). The survey will remain on-line
until Monday, January 12, 2004.

We strongly encourage you to share this with any other staff in your
offices, including student and hourly workers. Employees do not need to
be full-time employees to complete this survey. Likewise, please feel
free to share this survey with colleagues in other colleges who may not
have access to this e-mail.
Before you begin the survey, be sure to read the consent form at the
beginning of the survey. It gives details about your right to skip any
question and to exit the survey at any time. It also provides
information about the anonymous nature of your survey responses. At the
end of the survey is an optional final question which is not anonymous.
Answers to this question will be located in a website and server
separate from the rest of the survey. With this final question, you can
request a free summary of the survey results, ask for a discount on the
published final results of the survey, and/or volunteer to participate
in a follow-up spring survey from AHEAD. Answers to this single final
question are not confidential (AHEAD staff will have access to them),
but they will not be used for solicitation purposes and will not be
shared with anyone outside of the AHEAD office.
Please be aware that the data from this survey may be used by AHEAD's
deaf research assistant, Wendy Harbour, for her qualifying paper or
doctoral dissertation at Harvard University (under the supervision of
her advisor, Dr. Thomas Hehir and the Harvard Institutional Review
Board).
