Dear Colleague
The University of Wales is presently reviewing procedures with regards to
the replacement of certificates. This is a topic which has been discussed
on a number of occasions in the past but I should be grateful if you would
spare a few minutes to consider the following:
1) Currently we issue 'facsimiles' of originals certificates to students who
have lost / destroyed their originals. The only difference is that we mark
the new version with "replacement certificate." In all other respects i.e.
signatures, design and format, the replacement certificate matches the
original. We wondered whether this policy is in place at other
Institutions.
2) Were 'facsimiles' are not produced, we'd be interested to know what
procedure you have in place to issue replacement certificates. (Perhaps the
old info on the current design?)
3) We require students to pay a fee for the issue of a replacement
certificate when the original has been lost / destroyed. However, if a
certificate has been damaged and is returned to us, we then issue a
replacement free of charge. I wondered whether other institutions do
something similar.
4) We issue new certificates to students who have undergone gender
reassignment since graduation on receipt of the old certificate (and
relevant documentation confirming the change) and are unsure whether it
would be appropriate to charge a replacement fee in such cases;
5) Currently we will issue new certificates to students who have changed
gender only and I should like to know what the policies are in place at
other institutions with regard to students who have changed their name since
graduation either through divorce, marriage or for other reasons (e.g. deed
poll). Would you issue a new certificate to such people, and if so, would
you charge a fee?
I should be grateful if you would forward your response directly to me. I
shall be happy to circulate a summary of responses to those who wish to
receive them.
Many thanks for your time
Vicky
