I suspect both effects would co-exist, David. There may be reduced "demand"
(i.e. reduced attendance), but in those who do attend one might expect to
see increased expectations ('cause they're paying for it!).

Your CEO probably thinks he's working for a "health" service, John. Of
course, over here we know that we're not working for the National Health
Service at all; we're working for the National "Disease" Service! At least
we are in A&E, and most of the secondary care sector for that matter...
