I counted with interest the number of 'middle grade' posts advertised in
this weeks BMJ, for A&E - at least 22. How many of these will be filled? Who
is applying for these posts? How many 'middle grade' posts are now unfilled
in the UK.
Surely the whole system is on its knees.
'See and treat' is a fine idea, but we must look at the whole order of A&E.
Why, do A&E SHO's see medical patients and ref. to Medical SHO's, who'll
revist the history and examination?
Surely the model should be a central 'Atrium', with rapid assesment to
appropriate speciality, under the supervision of 'interested' specialists?
Now that training grades are working 'shifts', we must radically change the
way the system works.
I believe that 'training grades ' should be shared, and with the increasing
shortage of 'middle grades', I believe that this is the only model that is
sustainable.
Key words - 'duplication of work', 'sharing of staff'.
Please debate these points.
