I saw a chap last week with 'F*** me' inscribed around his umbilicus. I
ignored it without a second thought. Is there an ethical dilemma here that I
missed?

So far it would appear that witnessing or having a signature would make you
think of taking his inscription seriously... However, I suspect I would
take 200% of his consent prior to have the job done, so maybe witnessing
(the tattoist was there...) or signing it is perhaps superfluous.
The probably rethoric question is generated by another case when a woman
came with a suicide attempt, using metric tone of whatever pills she could
lay her hands on. Sobering up for several seconds during some resus she
yelled I WANT TO DIE, statement completelly ignored by the brave (read
poor) soul that was carrying it. She later sued him for battery and assault
as she clearly mentioned her wishes...
