saw a 12 year old last week
presented with what appeared to be oculogyric crisis
eyes deviated upwards, trismus and neck extension. He
had been given one Beechams tablet 30 mins earlier
(pheenylephrine, guaifenesin,caffeine, paracetamol)but
nothing else in last 24h.
responded promptly and completely to second 5mg of IV
procyclidine, therefore seemingly proving the
diagnosis.
Cardiff poisons could throw no light on it. I can't
see a mechanism for this myself with these drugs.
Can anyone explain this? Secret self medicating with
metoclopramide was enquired about but denied
convincingly. _
