Andrew:
The etiology could be several things. A small neuroma or periosteial
inflammation from a contusion or micro trauma seem most likely. If it is
superficial I would prescribe a course of iontophoresis with hydrocortisone
or DMSO.

The epicentre of pain is about 1 cm diameter.
Please (everyone) note - I got the first posting wrong - the epicentre of
pain is LATERAL to the anterior border of the tibia, but radiates onto the
anterior crest.
