At present, in our Department there are 4 Consultants and 7 Middle grades. A
5th part-time consultant has just been appointed. We see around 60-65k new patients
per annum.
I agree with Adrian Fogarty that it should be a requirement to have someone
experienced in the Department at all times, or else there will always be first
class (office hours) and second class (out of hours) medicine.

In Oxford we have a Consultant and/or a Middle Grade on the shop floor
seeing
patients and supervising SHOs 24 hours a day. It is more demanding than
being
on call, but it makes a difference (and the SHOs like it).
