Yes - I am still not an expert but it looks like the (uncosted and as
yet unnaccepted) contract sets out that there will still be primary care
responsibility 24/7. I am guessing but this may be the same sort of
'responsibility' that we have seen exercised when Health Authorities
(and possibly PCTs, PCGs and LHGs) have chosen not to provide 24 hour
community nursing cover - he hole left by which has been plugged mainly
by GPs and to a lesser extent by A+E ie the organisation takes little
responsibility if others will do the the paper and sticky tape on the
edges
This is all potentially divisive and can work against us. The BMA has a
role in co-ordinating our negotiations - I am not sure how effective
this is. We need to remember that we are not in competition with each
other but fighting for an adequately resourced system to provide the
high level care that we all strive for

(PS an update from BC - the govt took the physicians to binding
arbitration over pay - the govt didnt like the result (which was
physician favourable) - so they legislated the findings and the
arbitration process out of existence a few weeks ago. Some docs have
reduced services (against the advice of the BC Med Assoc) but the BC Med
Assoc has been back to the table with the gov - this failed to produce
agreement on Friday. There have been physician 'strikes' in BC before -
I fear we are heading this way again. Fortunately this involves all
physicians so there is a feeling of solidarity and camaraderie - this
feels much better than the UK system where the govt manages to get us
all fighting with each other and the BMA is not seen to be holding us
together)
