I frequently see patients with soft tissue restrictions post op TKA with
similar complaints as described. Stretching and soft tissue work usually
takes care of it. I would start my differential dx at the knee before
looking at remote structures such as the lumbar spine.

I might be interesting to find out how much knee extension they had pre
operatively. Is it that now they have a 'new knee' they are utilising
movement they did not have pre operatively and possibly causing a traction
injury or even an ischaemia of the nerve by repeated knee extension ?? The
order of the day might then be checking all appropropriate interfaces in the
leg and mobilising accordingly.
