Every health secretary since Enoch Powell has said that a higher proportion
of health expenditure should be in the community sector as opposed to the
acute. Every health secretary has spent a higher proportion in the acute
sector. In Maryland where they experimented with a referendum on health
priorities, they found a lot of support for spending more money on
transplants and care of premature babies at the expense of care of the
elderly, HIV care, addiction programmes etc.
