At Queens Belfast we experienced similar problems with
Frontpage and more recently XML code in WORD 2000.
Our official line is to use Dreamweaver (available through
Chest) which seems to give fairly clean HTML and also has an
option to clean up Word HTML.
On the other, even when we have clear Corporate Identity
Guidelines and give courses on good practice on design,
accessibility, usability etc. there will always be some means by
which sites can appear which completey ignore these.
