Hi Jane,
We're using SurveyMonkey at http://www.surveymonkey.com/ for feedback
from our workshops. It is exceptionally good and you can customise it
in a variety of ways, as well as have reports returned and sorted by
any mix of questions. You can use it free for Questionnaires with less
than 10 questions - worth using to try out its functions. Sorry about
going on - but it is a very professional tool.
