get a letter from the LA confirming how much you've paid for the year(s) you are
claiming (email their subs dept with your membership number, and ask them for
written confirmation of what you've paid for professional subs in the
relevant years ). Then send this letter to your tax office (details are usually
on your payslip, or from your personnel dept) together with your NI Number and
your details stating you want tax releif. This is what I did, together with a
letter telling them to contact me if they needed any further info. I got a
refund for a couple of years of tax on the subs I'd paid (whatever percent tax
had applied to my salary) e.g. something like 18-20 for each year IIRC (it was a
couple of years ago) and I think they adjusted my code for the current year.
