I would use the typesetting system LaTeX. If you want to
automate aspects of the form creation, in case you want forms with
questions in different orders etc., then I would write a perl script to
generate LaTeX source code.
Sounds like a lot of work, but it turns out to be less work than trying to
use other people's buggy software.

I've been using one called 'Modalisa' from Paris, France. It is easy to use
and does give you helpful statistics.
