If it is a formal discussion I record it in the individuals notes. If a
manage wants us to see an individual, for example for return to work or
about sickness absence, he fills a form in which he sendst to me. He makes
the individual aware of this. This goes in notes along with any emails.
Hearsay or gossip I don't record - but I do remember!.

Where do people document discussions with managers about individuals?
If, for example, we were reviewing someone who had work-related issues, and
the manager contacted us to give us some background to the problems (which
we obviously bear in mind but do not treat as gospel), would people document
the points in the notes ?
What if the management discussions had a significant bearing on the case
itself ? Where would the, "thanks for the info bear in mind but don't
record" turn into "better record this in case", and again, when it reaches
the latter, do we record this in the individual's notes or the managers
notes or somewhere else.
If it is recorded in the individual's notes, I take it from that, that we
need to inform the individual that the manager has contacted us to discuss
the problem. That being the case, we cant offer a private conversation with
the manager.
If we did not listen to the other side of the story, quite often the
versions of the events are quite different and its difficult guiding the
individual to the route of the problem, therefore this insight can be quite
useful. But! where do we record it?
