The Vale of Leven DGH had a problem with its Junior Doctor hours a few years
ago and instituted a policy that I thought worked very well (better than
sending the punters down to A+E at 4am!)
One experienced night sister was trained to do the junior doctor procedures
such as iv access, bloods, ABGs, 12 leads and male cath. They then
performed these for the medical and surgical house officers during night
hours.
The interesting part is that 2 hours per night were protected teaching time
set aside to instruct other nurses in the same skills - so they were
essentially 'auto-replicating' posts.
