If you've not got it already, try and get a copy of 'peripheral nerve
blockade' by CA Pinnock, published by Churchill Livingstone-it is an
excellent book (and fairly recent) and all the blocks really do work.
This contains detailed descriptions of the cubital blocks you are
using including pitfalls, and has sections on potential pitfalls
generally for nerve blockade etc. The end of the book has references
to further publications including upper limb blocks. I agree that
there is little out there in the way of publications, but hope this
helps.
