Re infection, you might consider a spider bite. There are a number of
spiders in SE Qld that can cause significant localised swelling. And as
someone else said, the knee problem might be symptomatic of a protective
gait, esp if the patient is elderly or unfit.
2.
Although a long shot, you might also consider a ruptured plantaris. However,
this is more associated with a very tight swollen posterior crural mm
compartment. This mm commonly spontaneously ruptures in people over 45.
Obviously, the pain increases with dorsiflexion.
