I gave tranexamic acid once when I was a med reg, back
in the thirties or whenever. A schizophrenic
presented with LBBB and repeatedly said 'chest pain,
chest pain' so I thrombolysed him on CCU. He promptly
produced a melaena stool. So did I.
anyway I rang a haematology consultant who said give
tranexamic acid and various blood products.
I asked the haematologists in Bath this question a few
months ago and they said the same.
Rather that write a protocol, the line here is that
you start fluid resus and get immediate haematological
advice - which you need to get your hands on the blood
products anyway.
Sounds like RUH Bath is on the ITV news at 6 tonight
over our lack of bedsand the effect on the E.D. 466
acute beds for 480,000 people. Hands up then - anyone
got less?
