There is sufficient evidence to show both ways round: social and
economic inequality (not to forget poverty) cause ill-health and
disability as much as the other way round. So what is the
controversy all about [with re to last e-mail from Mel B (hope not
one of the Spice Girls)]?
I can only speak from international health perspective. My own
empirical research in India shows that ill-health (and disability) is
ONE of the major causes of poverty. And according to UNDP most
of the disabled and chronically ill people live in poverty in
developing countries.
Secondly, I don't think Alex is against ALL health economists.
Probably he is pointing to the kind of health economics being
forced down our throat (CBA and efficiency-driven without any
regard to equity especially in LDCs, neo-classical and neo-liberal
interpretations of health by most international organisations?).
