An eccentric program would be as follows:
Over about 6 weeks. Start with very slow exercises, 3x10 reps, no pain (though possible mild dicomfort in last set is OK). Follow with ice. Once pain free, increase the speed to moderate levels. Remain at that level until pain/discomfort is nil, then increse speed again (ie. fast). When that is pain free, increase resistance. If pain is experienced at any level one needs to drop back to the previous level. This process may be repeated each week or so for 6 weeks, after 3 weeks you may add weights (external resistance).
This is an example - your own clinical judgement is the most important.
