Jackie,
I wonder if our department's guideline might be helpful?
I have attached it as a Word File
If nothing else the clinical guideline might allow your department to
discuss the issues and decide priorities.
I would also be pleased to know if anyone else on the mailbase could suggest
some improvements in the guideline. We have been using it for about 4 years
now and I would like to get feedback (so as to be sure we are not missing
something because we wrote it).
As for the financial workings - We worked out our budget for biomechanics
assessment, provision of temporary orthoses, permanent custom orthoses,
extrawidth/depth stock shoes AFOs and also bespoke surgical
footwear/appliances via the orthotist. The method was our provision and
referral pattern over the previous 5 years and if you would find it helpful
(I am sure we can resurrect the information).

I'm trying to find out what different trusts are currently doing regards
to biomechanics allocation.
What I mean by this is who can access biomechanics for an assessment?.
Who gets a prescription devices?. Do some trusts charge for the
device?. Do some trusts charge a nominal fee to help cover some of the
cost?. Do what manufacturers do trusts use to order of the shelf
devices?. Do trust find it financially beneficial to make there own
devices?. Do biomechanics specialists work on their own or as part of a
team?.
If anyone has any ideas or answers to the above questions I would be
very grateful. As the trust I work for has had no development in the
biomechanics service for a number of years and I'm trying to build up
the department. So ideas from trusts that already have a good system in
operation would be gratelly appreciated.
Thanks For Your Help
Jackie Ellis
