This conference will focus on the practical and strategic aspects of patient profiling, and how this can help deliver the NHS plan and the wider health agenda. Cost: £95.00 (inclusive of VAT).
WORKSHOPS
1. INVOLVING STAFF AND COMMUNITY:
Using the expertise of national pilot projects to provide practical examples of involving practice staff and local communities in patient profiling and in using the information provided.
2. PRACTICAL GUIDE:
A practice session on designing a data form to meet local requirements, collecting the data and storing it. Issues of confidentiality will be discussed.
3. MAINSTREAMING:
How to get Primary Care Organisations, Health Authorities and the Department of Health to take this on board and how to make this attractive to your budget holder? A national movement?
4. USING THE DATA:
How to make the data work for the individual patient, the practice, the PCG/T. How to create a culture of enquiry and of using the data within a practice.
