Subject: Replacement Degree Certificates

To help us at Sheffield, could colleagues please spare a moment to
advise on their procedures for providing replacement degree
certificates, in those cases where the original certificate has been
lost/stolen/destroyed, etc.
1. Do you supply a new 'original' certificate (ie bearing all the
appropriate signatures of the day and in the same format as the first
certificate)? 2. Do you supply a new certificate in your current
format, but including the graduate's original details (thus possibly
differing in format and appearance from the first certificate)? 3. Do
you supply some sort of certified statement, rather than another
certificate? 4. Do you have a different procedure?
Given the subject matter, I am sure that you may wish to reply to me
direct rather than via the list, in which case I will provide a short
summary in due course, if appropriate.

Summary of replies:
21 replies have been received, out of which 9 institutions issue
replacement certificates in the "original" format (ie model 1, above).
It is relevant, I think, that 5 of these are post-92 institutions,
where is it perhaps more reasonable to expect that blanks of original
certificates would still be available. Of the 9, two indicate on the
replacement certificate that it is a duplicate.
12 institutions issue replacements in the current format (ie model 2,
above) and 7 of these mark the replacement to indicate that it is a
duplicate. This is either by dating the new certificate, signing and
stamping it or including statements like 'this certificate replaces
one reported lost' (or similar).
Many respondents indicated that they also issued 'to whom it may
concern' statements, proving the award of a degree. These were
available in addition to the provision of replacement certificates and
in some cases also attracted a charge.
