Thanks to those who responded to this query and I hope I have interpreted these correctly. The responses were:
2 people saying they charged (a) an annual retainer to cover administrative costs plus ad hoc queries, (b) an annual, reviewable, cost per head based on the services offered plus local factors, (c) ad hoc item of service charges for work outside these boundaries. They did not disclose their fees but commented that much depends on local factors peculiar to the individual contract.
1 person operating a detailed item of service payment scheme.
1 person operating a scheme with (a) a £20 per head annual charge to cover administrative costs, ad hoc queries and some services plus (b) a reduced (at cost?) item of service charge for consultations and immunisations.
