I would be grateful for any ideas with regard to the following problem.
The patient history is as follows:
No history of injury
On a camping holiday 3 weeks ago noticed pain in knee on crawling, kneeling
and in the crouch position
The point of pain is very precise:
medial aspect of the knee
at 90 deg flex 1cm below joint line
1cm lateral to the medial border of the patella
On palpation it feels like a small lump (hard) that moves around about a few
mm. The sensation on palpation is pain scoring 6/7 on a visual analogue
scale. When there is no direct/indirect pressure on the spot there is no
awareness of it at all.
On palpation there is also the sensation that the lump is attatched all
across the knee (a narrow band passing laterally under the patella)
All conventional tests for ligament/meniscus etc injury are negative. There
is no swelling or inflammation apparent and there has been no joint locking,
loss of ROM etc.
I am at a loss! Is it possible to have a spur in this area?
Other ideas are a small cyst or scar tissue (the latter being unlikely as no
previous hx of knee problem).
Any ideas on diagnosis and/or treatment?
