To Mel and the rest of the list:
The best recent reference I know of is " Central pain mechanisms: A new
horizon in manual therapy" and "The clinical applications of central pain
mechanisms in manual therapy" by Michael Shacklock, both in the Australian
Journal of Physiotherapy 1999 Vol. 45.
This explains in detail why pain does not reflect the seriousness or
severity of disease. It is wonderfully referenced as well. This is the type
of writing that should generate a good deal of discussion in our community
for what it implies about our patients, the way we interpret what they say
and the way we treat them. But somebody's going to have to read it first.
