I'm not sure who should make decisions.
I work in a 'hybrid' Minor Injuries Unit/Urgent Treatment Centre/
Walk-In Centre in what is now a Community Hospital but previously was a DGH.
A builder carried in his unconscious colleague today. The patient was
immediately recognised by our Staff Nurse as being a diabetic who
suffered frequent hypoglycaemic attacks and usually attended the
hospital where she previously worked, a few miles away.
We then discovered no 50% Glucose in our Resus cupboards
No 50% Glucose stocked in Pharmacy
BM was 1.5mmol/l
ENP attempt at cannulation failed due to combative nature of patient.
I got a line in and ran in some 5% glucose and patient had 1mg Glycogen im
Pharmacy suggested two other units which might have glucose but did not.
Pharmacist eventually appeared with some 50% Glucose from Day Surgery
This resulted in rapid recovery. By this time Staff Nurse had popped
to the sweet shop for the MarsŪ Bars the patient always had on
recovery from a hypo. He was delighted to receive them!
I have filled in a Critical Incident form and this probably won't happen again.
I would still like to know who decided we no longer needed the glucose...
