In many cases the result probably wouldn't change the prehospital
management. Could the result influence triage decisions once the patient
arrives at hospital? Is it possible that a negative trop/CK-MB/myoglobin
(three-in-one devices are available) and negative 12 lead obtained in the
field mean that the patient may be triaged in a way that doesn't tie up
scarce resource such as monitored beds? Given that serial enzymes appear to
offer increased predictive value, couldn't a field test performed 30 minutes
(or in some areas here >1 hour) prior to admission represent the first of
the series?
