Another late response - I have come across examples of qualified DC
embedded in the source of the home page of the Internet Archaeology
e-journal at http://intarch.ac.uk/ and also attached to each article
within each issue.
The DC has been implemented according to the guidelines edited by Paul
Miller and Daniel Greenstein on behalf of the Arts and Humanities Data
Service and UKOLN.
Please note - these examples are not actually mine to offer but there
are contact details for the journal editor at the same address.

I have volunteered to help Diane Hillman add examples of qualified
Dublin Core metadata tags to the User Guide (to be renamed Using Dublin
Core). At the DC-7 meeting the User Guide Working Group discussed the
difficulty in finding good examples of qualified Dublin Core elements
directly from implementation sites or their user guides. In many cases
the metadata is stored in a repository and can not be viewed from the
Web pages and the guides rarely show the metatags marked up in HTML or
XML. I would appreciate any implementors sending me examples of their
qualified Dublin Core metatags in HTML or XML for inclusion in the new
guide. Please send me your examples by April 15th, 2000. I realize that
there may be some modifications based on the final decisions for the
proposed qualifiers. I want to thank Renato Iannella and Denis Stevens
for their response.
