sorry about the delay in my return re tri athlete with pelvic pain.
Wendy and Scott, I assessed this chaps psis asis in st: and sitt: in flexion and neutral and no height difference was noted. SIJ compression distraction and a central p-a on the sacrum did not cause pain or discomfort.
Owen, if you're out there i carried out your tests. He is an upright runner (pulls his c of gr)and a thomas 2 joint hip muscle stretch elicited pain and showed a tight ITB (tendency to prefer an abd position), I didn't note too great a tibial rotation. However, he does not exhibit an extended hip position h/ext knees. Rectus abdom appears okay and he does not have a rotating grtr trochanter during SLR or elicites pain. On all 4's hip/spine flexion = okay.Craigs test is still a mystery to me please send details.
Correct me if you wish but i believe this is what's going on.
Here's an athlete training to his max. His hamstring reaches the threshold and it tear at its musculo-tend junction. 6 weeks later feels good, but is it correct you can have no pain even though complete healing hasn't occurred, starts swimming 90 lengths!! feels good so begins jogging. The system is not up to it and 2ndary hyperalgesia sets up to protect the original injury. Now he has these 2ndary signs with what i can see are not matching up with tissue based testing. So we have a local hyperalgesia with a central sensitisation confusing the picture.
I plan to create a slow "paced exercise" routine and try not to focus too much on the pelvic inconsistant pain.
How long does it take to recover from an over training incident
Many thanx for everyones help and i'm still open for suggestions and correction!
