Although you state the problem is predominantly at
night - is she in a particular position? Does anything
aggrevate the symptoms? Does anything ease
thesymptoms?
And a throw away bit here - creeping/ ants crawling
sensations can (but obviously not always) be
associated with thoraco-lumbar fascial tightness/
immobility/ inelasticity or even fascial problems such
as these in the leg (- aside from any neural or other
causes of course).

I considered fascial origins first. She has some trigger points in the
ipsilateral gluteus minimus/medius. She also has a Grade I spondylolytic
spondylolisthesis of L5 but I don't know that there would be a correlation.
As for precipitating causes, the phenomenon goes back to early childhood and
there is no memory of anything. I don't know if the onset is positional or
not but once the sensation begins the only thing she can do to deal with it
is to just let go and try to relax which isn't very effective, or pace around
the house in the middle of the night.
