The BUMPS midwives in Leicester take some of their women from direct
referral e.g. women they have previously looked after who are pregnant
again, but the arrangement is they then notify the GP who sets up the
maternity care payment from the 'Red Book'. Its losing that bit that
threatens GP's.

I'm sure this is done in many areas. The One-to-ONe midwives at
Hammersmith and queen charlottes Hospital in West London have direct
referral from women as well as from GPs. this is particularly common
from women they have cared for before and women can often be booked for
care with the same midwife(ves) again. As far as I know the south East
London Group Practice have a community-based clinic where women can
walk in for info. or to book for care. they also work on a caseload
model of practice. I also know from personal experience that if you
really want to and know your way around the system you can do this for
yourself - with my fourth child I simply rang up the community midwife
who had cared for me with my third child and fixed up visits with her,
while my GP simply wrote the official hospital referral letter for the
records. I had to go to the hospital for a booking visit but thereafter
just saw the community midwife.
