In Ontario, the midwives do all the care for women. We have a set of
guidelines for mandatory consultation and transfer of care throughout
pregnancy, birth and postpartum. Your midwives may need to improve their
physical assessment and PAP skills, but it works very well.
We have a Swedish midwife practicing here in Ontario and I am sure she
would be a valuable asset to your project. I will contact her and see if
she is willing to provide some support.
The gynecologists will need some time to get used to the concept, but it
should not be difficult as midwifery has been long established in Sweden.
We did not get legislated until 1994, and it is going well. The midwives
practice as primary health practitioners. You might look into the program
set up by Leslie Page, "One to One Midwifery".The report is available. The
following is an advert for midwives to join the programme.

>>At Queen Charlotte's & Chelsea Hospital, we don't take the usual
>>detached view of midwifery.
>>
>>Our new One-to-One Midwifery Practice turns normal practice on its
>>head.
>>
>>Basically, it puts a named midwife with every woman, planning and
>>setting the standards of care for an individual mother throughout her
>>pregnancy and right up to the birth. (An amazing 80% of births being
>>attended by her own midwife).
>>
>>Each midwife works in a pair (continually in touch by mobile phone) so
>>if you can't be there your partner will.

>>MIDWIVES
>>Grade F £20,818 - £24,837
>>(ref: W/44)
>>Grade G £24,032 - £27,292
>>
>>
>>Often the midwife is the lead clinician although, when needed, there's
>>still the option of GP or consultant led care.
>>
>>Early results from our evaluations are amazing. We've had more normal
>>deliveries, less use of epidurals, fewer episiotomies and most
>>importantly both mothers and midwives think it's absolutely brilliant.
>>
>>In fact it's been so successful that now we need more midwives to
>>expand the programme.
>>
>>If you want the responsibility, the feeling of being a professional
>>pioneer and the immense job satisfaction, you too could be part of the
>>One-to-One practice.
